It's all about money

Linden lab is not interested in doing anything unless it involves increasing their profit. Most, if not all, of the major changes and sudden policy adjustments can be traced down to things that bring about potential monetary loss to Linden Lab.
I to have taken note of how so much of the system is completely plagued with bugs and errors, but the smoothest running part of all of Second Life is anything that involves money going *inbound* to Linden Lab. They've made darn sure that works perfectly.
All the issues mentioned are valid problems that need to be solved. However they do not cost Linden Lab enough money yet to make them even ponder making an effort to change.
